When Requesting
Letters of Recommendation
from Dr. Cannon

Follow this checklist:

___ Provide a list of Psychology/Neuroscience courses you have taken, along with the grades received and the professors' names.

___ A listing of all schools you are applying to, the deadlines, whether or not there is a form to be completed (yes/no), what program you are applying to for each school (clinical, counseling, MD, OT, etc.), and whether it is a doctoral or master's program.  Important!!  The deadlines you list should be the dates when the letters can be placed in regular campus mail for transfer to the US Postal Service. 

___ Curriculum vitae

___ GRE scores (if available); if not, please include SAT scores

___ Overall GPA and psychology (or neuroscience) GPA

___ If you've done research, list the faculty member(s) involved, a brief description of the project, whether it has/will be presented somewhere, and your intellectual/physical involvement in the project. 

___ When/How you first met Dr. Cannon – calculate how many years/months Dr. Cannon has known you - it makes for smooth writing to know the years/months. 

___ Provide addressed (or named - see further) envelopes for ALL letters. Even if letters are to be returned to you (rather than sent directly to the school),  include a self-addressed envelope.  Obviously, letters that should be returned to you should have your name, or address, on them AND the name of the school that it's intended for.  

___ Due to Dr. Cannon's notoriously bad handwriting, please type in all demographic information (for BOTH you and him), except the date, on the forms to your schools. When asked for name and position of professor, type (if it all fits):

___ Create a Pendaflex folder for yourself and place it alphabetically in the Recommendations Drawer.

___ On the sheet of paper hanging from the bookshelf by the plants, write your name on the first date when you want Dr. Cannon to get your letters out - not get them there - get them out.
