Health Information Requirements and Medical Records

Student Health Services maintains health records on all undergraduate students and those graduate or part-time students who opt to access our services. Complete and up to date medical information is vital in the provision of quality health care to students. Students are required to submit a Health History form that includes general information, health history, physical examination record and immunization record. (Form may be downloaded here or obtained from Student Health Services.) Health records are maintained during time spent as a University student and for seven (7) years beyond the date of graduation or the last visit, which ever comes last.
