2007 Application - To be completed by individuals desiring admission to the PVAA. Completed application and two recommendation letters accepted until class seats are full. 

2007 Scholarship Interest should be submitted in writing with the PVAA application.

Accepted Students:

Undergraduate Academic Credit - To be completed by those admitted to the PVAA desiring 3 undergraduate academic credits. Completed paperwork and payment due May 1, 2007.

Scholarship Award Confirmation - To be completed by those admitted to the PVAA and who were awarded a full or partial scholarship. Completed acceptance form due May 15, 2007.

PVAA Advisory Working Group Members:

Expense Reimbursement - To be completed by Advisory Working Group members seeking reimbursement for travel and other authorized PVAA-related expenses. Completed form should be submitted within two weeks of incurring expense(s) and be accompanied by ORIGINAL receipts.
