Application to Graduate School
Used to apply for admission to graduate school.

Personal Data Sheet
To be completed during initial interview.

Transfer of Credit
Used to request evaluation of courses for transfer into program.

Course Registration Form
Used to register for courses.

Schedule Change
Used to change a course(s) after registering or to change name, address, phone number.

Reader Form
Used to register for internship or directed study. Please see Program Director.

Application for Degree
To be submitted when registering for the semester in which the student plans to graduate.

Application to Defer Tuition Payment
Used to defer payment of tuition based on employer reimbursement.
